If you prefer to complete these forms in person, please arrive 15 minutes prior to your scheduled reservation so you can relax in our patient lounge.

To enhance the patient experience, we have designed a comfort menu. We offer noise-cancelling headphones, warm lavender towels, neck and back support, access to the doctor’s cell phone, and the comforting presence of our office pup, Hank. The use of digital 3D scanners ensures greater precision and comfort, while the latest advancements in AI and smart technology further elevate our services.

Our doctors will curate a personalized plan, presenting all available options and answering your questions. We encourage you to bring a list of your dental health concerns and goals for your smile so we can address them together.

Please arrive with a valid photo ID, your current dental insurance card, and a list of your current medications so our team can best support you.

Where to Park

We offer exclusive patient parking price reduction at the Hyatt Centric Garage (1 block away at 1620 Chancellor St). With our sticker, your rate is $16 for up to 2.5 hours.

Scheduling Courtesy
